When performing a killing blow, the appropriate action is to place the tip of your weapon at an angle, lightly resting on the target's back or shoulder. No significant force should be used to hold the weapon against the target of the killing blow. Vertical or horizontal positioning of the weapon is discouraged.
To stop a killing blow, a third player may knock the weapon away from the target, and once contact between the target and the weapon is broken, the killing blow is interrupted. Healing the target will end the killing blow. Incapacitating the attacker in some fashion will end the killing blow. Dragging the target away from the attacker will end the killing blow.
It is important to remember that the game mechanic to stop a killing blow is not necessarily the in play visual, the game mechanic is just the safest OOP way to signify the action.
Adding a descriptor to the end of the killing blow count, such as "ripping off head, " "snapping neck," or "disemboweling" is welcome. I suggest the use of moderation in this practice though, as a "flavor" phrase repeated ad infinitum can get tedious.
In the case of monsters with particular weaknesses like trolls and werewolves, killing blow descriptors such as "fire" and "silver" (respectively) may be used every time.
